Come with a clean face, freshly washed with no makeup. It is best if I see your skin in its natural state so I can recommend a customized plan.
Know what products you are currently using on your skin, including product names and brands. Write them down and/or bring them to your appointment.
Fill out your pre-appointment paperwork on the portal (link sent to your email with appointment booking). Knowing your relevant health information and signing your consent forms will speed up the check in process and allow us more time to discuss more important topics-like our plan of action.

Yes! We use Allē and Aspire loyalty rewards for Botox and Dysport treatments. Please sign up at Alle.com and aspirerewards.com ahead of your appointment, it’s very easy. I will be able to view your account at your time of service and add your treatment or use rewards in your account towards your purchase. Enjoy the products and treatments you love-and get rewarded whenever you do.

Recognizing that everyone's time is valuable, we ask that you provide a 24 hour notice when you are unable to keep your appointment. If less than 24 hours notice is given, the card on file will be charged a $100 cancellation fee per service booked that day. If you are more than 15 min late, you will be charged the $100 late cancellation fee and asked to rebook. We work hard to make sure we are all set up for your appointment and late cancellations often lead to wasting valuable supplies. No shows will be charged $250. Thank you for understanding. Evergreen Aesthetics is one room, with no waiting area, so we are unable to accommodate more than two clients in the space at any given time. Please be on time for your appointment so we do not have to adjust your appointment time to be ready for the next client.
